JNF Discharge
The administration and staff of this hospital are committed to a belief in comprehensive continuity of care to all patients. This means that from the moment you enter the hospital we will be planning together for the return to your home in the best possible state of health. Information will be provided pertaining to community services that will help you receive and maintain continuity of care.
The Admission and Discharge Planning Office is available to assist you and your family during your Hospital stay and in making plans for the care you may need after discharge.
Upon the doctor’s instructions to discharge you home, the staff will provide you with discharge instructions, diet, follow-up care, medication, daily living activities and any other special instructions.
Please make arrangements for your transportation home. If your transportation is delayed, you will be escorted to the lounge where you can wait.
Patients are responsible for the payment of any outstanding bills. These charges are to be paid to the cashier at the information desk/window at time of discharge.
